Our Services

True North Facilitation supports children, teens, and their families through every step of their Self Determination Program (SDP) journey with the San Diego Regional Center.

Services include:

  • Person Centered Planning

  • Support transitioning into SDP

  • Ongoing Independent Facilitation


The State of California has approved funding to support eligible Regional Center clients in transitioning into the Self Determination Program. This means that the Person Centered Planning process, as well as transition-related support and advocacy, are offered at no cost to your family. There is no out-of-pocket cost and no funds are taken from your individual SDP budget.

Person Centered Planning

This process uses the client’s vision for their life, along with input from their circle of support, to create a written plan. The plan reflects the client's goals, needs, and preferences in areas such as:

  • Friendships and community

  • Recreation and transportation

  • Therapies and services

  • Home and family life

  • Education and employment

Support includes:

  • Pre-planning meetings

  • Person Centered Planning Collaborative Session

  • Person Centered Plan development and completion

Cost: Funded by the State of California. No out-of-pocket cost. No cost from your individual budget.

Transition into the Self Determination Program

We provide Independent Facilitation to help guide your transition into SDP.

Support includes:

  • Presenting the Person Centered Plan

  • Identifying unmet needs

  • Working with the Regional Center team to develop an individual budget

  • Creating a spending plan

  • Connecting with a Financial Management Service (FMS)

  • Identifying and coordinating services and supports

  • Advocacy during the IPP meeting

Cost: Funded by the State of California. No out-of-pocket cost. No cost from your individual budget.

Ongoing Independent Facilitation

Once enrolled in the Self Determination Program, we offer continued support as needed.

Services may include:​

  • Revising or resubmitting the spending plan and individual budget as needed

  • Identifying and coordinating services, staff, and resources

  • Preparing for the annual IPP, budget, and spending plan

  • Coordinating with the FMS

  • General advocacy and support

Support levels vary depending on individual needs.

Cost: Paid through the client’s individual SDP budget. No out-of-pocket cost.
